Ashwagandha now has a more substantial human evidence base than many botanicals marketed for stress or cognition, but evidence quality should not be confused with product interchangeability.

Trials have used root extracts, root-and-leaf extracts, whole-root preparations and proprietary standardized extracts. A positive result from one preparation does not establish that another commercial preparation will reproduce it.

The 2026 mental-health meta-analysis strengthens the pooled signal for stress, anxiety and depressive symptoms, while a separate 2026 cognitive meta-analysis found significant pooled effects in several cognitive domains.

Those findings deserve acknowledgment. At the same time, heterogeneity, variable botanical reporting, different populations and evidence certainty ranging from moderate to very low mean the results remain more nuanced than a claim such as “clinically proven Ashwagandha.”

Safety deserves equal weight. Ashwagandha should not be described as harmless simply because it is herbal.

What Is Ashwagandha?

Ashwagandha is the common name for Withania somnifera, a botanical with a long history of use in traditional Ayurvedic medicine.

Modern supplements commonly contain extracts from the root or combinations of root and leaf. Whole-root preparations are also available.

Ashwagandha contains multiple phytochemicals, including steroidal lactones commonly known as withanolides.

Withanolide content can help characterize a product, but a higher percentage does not automatically mean greater clinical efficacy.

Ashwagandha is commonly described as an adaptogen.

A 2024 systematic review and meta-analysis included nine randomized trials involving 558 participants.

The pooled analysis reported significant improvements in Perceived Stress Scale scores, anxiety-rating scores and serum cortisol compared with placebo.

A newer 2026 systematic review and dose-response meta-analysis expanded the evidence base to 22 randomized trials involving 1,391 adults and reported favorable pooled effects on stress, anxiety and depressive symptoms.

However, the 2026 authors explicitly called for higher-quality trials because of substantial heterogeneity and uncertainty about optimal dose and treatment duration.

The most defensible conclusion is therefore that Ashwagandha has a credible and increasingly substantial human evidence signal for perceived stress, while the magnitude of benefit and applicability to a specific commercial extract remain uncertain.

What About Anxiety?

Several randomized trials and modern meta-analyses report favorable changes in anxiety-rating scales.

The 2026 mental-health meta-analysis strengthens the argument that there is a meaningful pooled anxiety signal.

That still does not establish Ashwagandha as a treatment equivalent to evidence-based therapy or medication for an anxiety disorder.

The trials differ in populations, formulations, doses, duration and measurement scales, and the 2026 review itself emphasized heterogeneity and the need for better-designed studies.

Why NIH wording can appear different: The NIH Office of Dietary Supplements health-professional fact sheet states that Ashwagandha extracts may help reduce anxiety and stress and improve sleep, while also emphasizing that different preparations and doses make specific recommendations difficult.

The NCCIH consumer page, last updated in March 2023 before the major 2026 meta-analyses, uses more conservative language and describes the effect on anxiety as unclear.

The evidence has therefore evolved, but it remains more accurate to describe Ashwagandha as promising for anxiety-related symptoms rather than as an established anxiety-disorder treatment.

What About Depression?

The 2026 mental-health meta-analysis also reported a favorable pooled effect on depressive-symptom scores.

This is scientifically relevant but should be interpreted particularly carefully.

A pooled improvement in symptom scales across heterogeneous Ashwagandha trials does not establish the supplement as an antidepressant or as a treatment for major depressive disorder.

Clinical distinction: Depression is a medical condition that may require professional assessment and established treatment. Ashwagandha should not be used to replace appropriate mental-health care on the basis of a supplement meta-analysis.

Does Ashwagandha Improve Sleep?

Sleep remains one of the better-supported Ashwagandha research areas.

A 2021 systematic review and meta-analysis included five randomized controlled trials involving 400 participants.

The pooled analysis found a small but statistically significant overall improvement in sleep with Ashwagandha extract compared with placebo.

Effects appeared more pronounced in participants with insomnia, in studies using at least 600 mg/day, and with intervention periods of at least eight weeks.

Those subgroup findings should not be converted into a universal dose or duration rule because different preparations were used.

Trials have reported changes in outcomes such as sleep quality, sleep efficiency, total sleep time, sleep latency and awakening after sleep onset.

Evidence calibration: Some Ashwagandha preparations may modestly improve aspects of sleep. That is different from calling Ashwagandha a guaranteed sedative or an established treatment for insomnia.

Does Ashwagandha Lower Cortisol?

Some randomized trials and pooled analyses report reductions in serum cortisol alongside improvements in perceived stress.

For example, the 2024 stress-and-anxiety meta-analysis found a statistically significant pooled reduction in serum cortisol compared with placebo.

This provides a biological correlate for the stress findings but should not be overinterpreted.

Cortisol is a normal and necessary hormone. Lower cortisol is not automatically healthier, and a supplement should not be marketed as universally “balancing cortisol.”

The more scientifically accurate statement is that selected Ashwagandha preparations have reduced cortisol in controlled human studies.

Does Ashwagandha Improve Focus or Memory?

This section has changed meaningfully in 2026.

Older evidence summaries reasonably characterized Ashwagandha’s direct cognitive evidence as limited.

A May 2026 systematic review and meta-analysis in Frontiers in Pharmacology now provides a broader pooled analysis.

The review included 20 randomized controlled trials involving 1,249 adults across cognitive and physical outcomes.

The pooled memory effect was moderate, while the attention and processing-speed effect was statistically significant but smaller.

The authors also reported a significant pooled executive-function effect, although the GRADE certainty for that outcome was low.

These findings make it inaccurate to say there is no meaningful direct cognitive evidence for Ashwagandha.

However, they still do not establish a universal nootropic effect.

The review identified differences in extract composition, plant parts and withanolide reporting, and the overall certainty across outcomes ranged from moderate to very low.

Global cognition came from only one study, and the visuospatial analysis was sensitive to the removal of a heterogeneous study.

Ashwagandha Dosage: Is 300–600 mg the Right Amount?

There is no single universal Ashwagandha dose.

The NIH Office of Dietary Supplements review documents substantial variation across trials evaluating stress and anxiety.

In one reviewed evidence set, extract doses ranged from approximately 240 to 1,250 mg/day. A separate whole-root preparation provided 12,000 mg/day of granules, equivalent to about 6,000 mg of root powder.

Several commonly discussed stress and sleep trials use approximately 300–600 mg/day of particular standardized extracts.

The NIH review also notes that some studies observed larger stress benefits around 500–600 mg/day than at lower amounts.

That does not establish 300–600 mg as the correct dose for every Ashwagandha product.

Dose cannot be separated from preparation: 300 mg of one concentrated extract is not necessarily equivalent to 300 mg of another extract, a root-and-leaf preparation or ordinary root powder.

The 2026 cognitive meta-analysis similarly found study doses ranging from 225 to 675 mg/day of Ashwagandha extracts, with 600 mg/day commonly represented.

Again, that is descriptive research information—not a universal dosing instruction.

NCCIH also advises against Ashwagandha for people with autoimmune disorders and for people who are about to undergo surgery.

Hormone-Sensitive Prostate Cancer

NCCIH notes evidence that Ashwagandha may increase testosterone levels and advises that people with hormone-sensitive prostate cancer avoid its use.
